Just to add my "true confessions" to this discussion...
I have used water various times with infants - but not as "feeds" . rather
as a way to initiate a suck-swallow pattern. If mom has difficulty with
let-down, especially when getting baby back to the breast after use of
bottles or when using a nipple shield (and who can have a let-down easily
with a Lactation Consultant, a Public Health Nurse, a family member and a
fussy baby all looking at her?). I find that if I use a syringe and put
mother's milk inside the baby's mouth by using a syringe alongside the
nipple or into the shield if baby has had a lot of bottles prior to my
seeing them. When the baby sucks once, swallows and they then start
suckling, this usually triggers a let-down and subsequent swallows without
any more help (or water). At times mom has no expressed or pumped milk, so
I use water. I consider this water use as "priming the pump" rather than as
a feed. Usually it's VERY small amounts.
I also have used water in a cup for babies who sound "gaggy" after cesarean
deliveries, and refused to breastfeed...just gag and gag. They usually have
a lot of mucus in their stomachs, and I found that if I gave them some small
amount of water in a cup or while sucking my finger and using a syringe,
they either threw up . then nursed well - OR they seemed to "digest" the
water and mucus and then ate well in 30 minutes or so.
I never considered these as "feeds" but as a way to get to the feeding...
NONE of this is "evidence based" - they were just tricks I found worked for
me.
