I use an assessment form, feeding observations, and teaching checklist for each dyad. This is not a part of the client medical record. I keep it in my files. The checklist I use goes something like this (mine is at work now)
____Usual breastfeeding patterns, contact phone no.
____Positions/latch
____Effective vs ineffective feeding
____I&O/Diaper counts/weight patterns
____Expressing/storing/transporting milk
____Diet/meds/illness
____Preterm/sick infant
I teach and have a handout that talks about best practices like rooming-in, STS, no bottles. You could have a check-off for "best practices." You could have one called milk supply, but my best practices one talks about building a good supply. I don't specifically go thru teaching about sore nipples (rarely have it) but I do have a handout on engorgement prevention and expressing. If you cover specifically cover that you could have "overcoming problems" or "engorgement/soreness." I have a line that says breastfeeding aids and I write in if I've used nipple shields (rare) or larger flanges or an Elite pump or referred to WIC for a pump loan etc. I also have a small box for notes, so if it's a premie I can mention some specifiics. Anyway, I like my checklist as it pretty much covers what I teach moms prior to hospital discharge.Laurie Wheeler, IBCLC, MN, RN MISSISSIPPI, s.e. USA
