Anne is correct in what she says - and I'd like to add that when we watch
the machine for the desats, we need to remember that there is a delay in the
information coming out.  So if the baby desats to 80 - that already happened
and when we see it the baby is in the process of trying to catch up.
Parents (and apparently some HCPs?) need to be able to WATCH the baby for
signs of stress and feeding induced apnea BEFORE the SAT levels drop like
that.  

 

One would think that showing the research (of which there is plenty) would
certainly help protect these infants.  The safety of the feeding is
critical.  The other thing to look at is how the baby is handling the
feeding - are they working so hard to feed that they end up burning calories
and therefore lose weight?  I am working with a baby (34 wkr now at 38 wks)
whose grandma decided that not pacing the feeding (as I and babies mother
instructed) was perfectly fine and baby has lost 2 ounces in the past 3 days
since grandma got here and started helping with supplementation.  Baby does
well on the breast but intake is low - so we supplement.  I asked grandma if
baby is breathing in between suck swallow bursts and she said "No, I lay her
back and cradle her all snuggly and she just sucks the bottle down in about
5-10 minutes - gulps like crazy, breathes real heavy and fast about halfway
through and then finishes up and sleeps for 3 hours."  She thought that was
a good thing!  So - we'll be seeing them today.

 

Don't even get me started on the reflux that I see due to babies eating too
much too fast.

 

Hopefully someday these types of situations where people stay fixated on
doing what they are used to (regardless of how unsafe it is and how the
research proves that other protocols are much more effective) will change
and people will be more open to reading the research, giving it a try and
being open to new possibilities.
