I don't think they don't think they are feeding the baby at the  
breast.  They are just not confident about being able to produce  
ENOUGH milk. They also may not realize that the breast continues to  
produce milk while the baby is feeding. So they may think it is  
"empty" (and hasn't that term been used to them?) after they fed the  
amount of time they usually feed the baby, and so they think they  
have to give the baby something else if he/she still seems hungry.
