Judy,
I quickly scan the mother's and baby's charts for any pertinent med/surg
hx, gravida/parity status, feeding record to the point of my visit, note
any supplementation, note delivery information, diaper output to that
point, wt hx, and ask the couplets' RN if she has any information
pertinent to the bf. this can be done in about 5 minutes usually.
any prior bf hx will be collected from pt in the room as well as
anatomical assessments, and bf assessment.
