"At this point, I would expect a
3 month old baby to probably be eating 32 ounces of formula per 24 hour
day -- in our culture maybe feeding more (more is better, right?). If
mom is nursing say 3 times but bottlefeeding 32 ounces a day, baby is
probably not taking in much breastmilk."
One of my most satisfying consults was the mother of a 4 month old who was taking 900 ml of formula a day, despite bfing and pumping 6x/day. Mom came with breast pads (leaking at 4 mo?), and baby's intake at breast was 100 ml! Turns out mom was "bfing and pumping, 6x/day", not "bfing, and pumping 6x/day". When she had been told by someone months earlier to build her supply, she had mentally misplaced the comma! The baby was spitting up a good bit of the formula, but had adapted to consume vast quantities of food at those 6 feeds per day.
Within a week, formula was down to 350-450 ml, even though mom discontinued pumping. The change? Increased nursing and paced bottle-feeds. Formula continued to drop to zero. For some wonderful reason, this mom's milk supply had continued to be strong and responsive, despite active (though unwitting) suppression for 4 months.
