Still haven't solved the problem, so am forwarding this, with another amen
from yours truly. Anyone who has labor-sat with a primip would agree with
this, not to mention how nice it is for a baby to have siblings from the
start :-)

My solution to this, when I become an all powerful ruler, is to have mothers
have their second baby first. Second time around mothers become much more
aware of what they want and they get it.
I'm not even going to blame the medical/midwifery people for this problem.
I've taught many an antenatal course and lots of first time mothers are
totally happy to abdicate everything to their care-giver.
They don't want to have an opinion.
