Debbie, in my area, I have a great breast doc who does ultrasound guided  
aspirations.
 
As for how to rule out an abscess, yes, she should expect an  ultrasound.  
Actually, I would say should be sure she has one. I have had  doctors try to 
treat with antibiotics without sending mom for the ultrasound,  and every single 
time the mom ends up needing the ultrasound and drainage  anyway.  Not sure if 
it's because the mother waited too long for the  antibiotics to do the trick 
or if antibiotics are just not strong enough to take  care of it  The nice 
thing .... if it is an abscess....as soon as it is  drained she will have some 
relief.  I have seen a breast go from rock hard,  bright red hot to milk being 
able to be expressed, the redness gone, and comfort  just hours after an 
aspiration of the abscess.  
 
If you search Lactnet's archives you should find some information about  
getting rid of an abscess naturally.  I have had success with this  too.  I have 
some clients that will not go the antibiotic or doctor  route.  They prefer 
homeopathic medicines their midwives advise them to use  and combine it with the 
potato poultice or castor oil poultice.
