When moms have *really* eliminated dairy and there is no improvement, 
they have a couple of options.  Whichever they choose, it would be very 
wise to improve mom's own digestion and gut health by taking probiotics 
and digestive enzymes.

The easiest elimination diet is to drop to four foods.  For a 
USAmerican, that generally means turkey or lamb, millet, pears, and 
squash.  A week or two on that will yield some very obvious answers, 
but a mom who is skinny to begin with may not like the weight loss that 
comes with eating only boring food ;)  It is mindless, though, and 
millet is quite edible if seasoned with salt and turkey fat.

For more food options but requiring more thought, is eliminating all 
major allergens: gluten (most common grains), dairy, corn, soy, egg, 
nuts, tomatoes, citrus, berries, and seafood.  This means no processed 
foods whatsoever, and it still may not fix things, because if the baby 
is severely sensitized to dairy, beef can be a problem.  Some kids 
can't handle rice if mom ate a lot of it, etc.

Both my children required the four foods diet, and then I transitioned 
to no major allergens, and by about a year of age I could eat whatever 
I wanted.  I could "cheat" now and again at six months.
