Hi, I have a mom I worked with 4 months ago when her baby was a 
newborn.  Baby is now just about 4 1/2 months.  Mom pumps occasionally, 
and reports that at previous pumpings, milk has been thick and creamy.  
Today she felt engorged for the first time since the newborn period, and 
so she pumped--milk was thin and watery.  This alone did not concern 
me.  She said she usually pumps about the same time of day, including 
today.  I asked her how she was feeling otherwise, and she said she is 
having central pelvic pain and pain at her incision area (c-section).  
The pain goes from barely feeling it to waking her at night, and keeping 
her from sleeping.  She has no fever, and no other symptoms.  Her 
midwife has requested she get an ultrasound and blood work including 
pregnancy test.  There is a possibility that she is pregnant.  No 
symptoms of a UTI, but urine has been more yellow lately. I suggested 
some cranberry juice and pills, and to increase water intake.  Baby is 
gaining well, is 17 pounds at 4 1/2 months, and is developmentally 
advanced, so all is well there.  He has more than enough poops and pees 
and his urine is clear.  Baby has been fussy lately and nursing more, so 
I suggested a growth spurt.  Also, maybe a return to fertility 
(pregnancy would indicate this, obviously).  Does anyone have any other 
ideas?  Mom knows I'm posting.
