OK - based on this, I don't see what is wrong with this baby?  Weight 
gain is good and output is normal. I would want to check this  mother 
is feeding responsively and not timing or scheduling feeds. I would 
want to observe a feed and  perhaps make some slight 
amendments/adjustments if necessary, but I hope all I would see would 
enable me to encourage this mother and build her confidence so she 
stops the frequent pumping and record-keeping and just enjoys her 
baby, holding him ad lib and being close day and night.

This sounds like an ordeal for everyone.

Could the mother not be supported to just go with the flow for  a 
week, lots of skin to skin and ad lib feeding? Then review after that 
time?

Give the mother and baby the chance to enjoy each other and let 
breastfeeding happen as part of their relationship and closeness.

I don't understand the rush to fix the situation, which I don't see 
as 'broken'  anyway, and which is certainly not one of clinical 
urgency....unless I am missing something.

I know the test weight showed poor transfer - at that time, at that 
feed, on that occasion.

Overall, for her to have gained a pound in probably two weeks, and 
for output to be normal, she must be doing just fine....surely?
