I'm working with a mom who is having difficulty latching a 3 day old baby
girl. Mom is early 30's, 5/5, breastfed her first 4 (2 were preemies and
she did very well breastfeeding each of them). I visited her early on day
1, baby was latching and suckling well at breast, mom very happy and
confident. She had contx on and off for 2 days before coming into hospital
at 7am. She had an epidural (with relief) at 7:30am and delivered vaginally
at 9am. Baby was taken to nursery at 28 hours for newborn screening, then
returned to mom and no latching since then. Nurse assures me "NO" bottles
given in nursery. Worked with mom and baby thinking baby just needed a
little extra encouragement. Oral assessment within normal, mom's breasts
larger but baby handled her anatomy fine in the first 24 hours. Baby roots
all over, latches, few suckles, then comes off fighting and arching even
with gentle continued assist, utilizing several positions and approaches.
Expressed milk and cupfed almost an ounce. Offered all the usual
suggestions for skin to skin, frequent feeds, cupfeed if no latch, protect
milk supply, bathe with baby, etc. (No complications for mom or baby during
labor and delivery or hospital stay.) Mom called today (day 3), very
frustrated after continuing with all suggestions. Continues to pump (great
supply), cupfeed and may try "paced bottlefeed" with big dome nipple but
really wants to get baby to breast. Consult set up for Monday.
Any ideas about why this baby did so well and now refuses and fights the
breast?
