I agree - not a concern. We know that the watery/fattier components
of breastmilk are related to the amount of milk in the breast. A
full, engorged breast is likely to produce on expression a
comparatively more watery milk.
It's not clinically significant in any way, and in fact it's
normal. She may have gone longer between feeds for some reason, and
this led to the engorgement. In the rest of your post you describe a
baby who is gaining and thriving, so there is no reason to think
anything is wrong.
I can't comment on the other parts of your post about the pain at her incision.
