Hi Sherry

Have you assessed the baby's labial frenulum?  If the baby's labial  
frenulum is tight the upper lip may not flange properly causing the 
teeth to rub against the nipple.

 From personal experience, my son had this problem.  The frenulum went 
right down between his front teeth.  Nursing always felt like sand 
paper was rubbing on my areola.  His front teeth made dents in my 
areola.  When I asked the dentist, he said he would cut the frenulum 
when my son was older.  He obviously knew that an active child will cut 
his own while playing because he didn't have to have it cut, but it 
sure did bleed!
