Hm, "over the top" originated, I think, from the First World War, where the
 rank-and-file would go out over the top of the trenches, most likely to
 injury or death.

 The "commanders" would certainly not be going "over the top", but were 
quite
 content for the ordinary soldiers to do so, despite the risks.

 Any similarities?  I think that "over the top" is a perfect description for
 where the babies are going.....  Though I know of course that it was the
 doctors who were saying that they were NOT going "over the top" with you
 into the minefields of Choice and Consumerism.
