Hello everyone,
I am officially stumped and would love some feedback. I have a lovely
patient who has a 6 week old baby who has been battling with a obvious and
nasty case of thrush. Mum had all of the classic symptoms of ductal
thrush: stabbing chest pain at the ends of feeds, nipples looked normal
though. Baby had one of the whitest tongues I have ever seen. We tried
treatment first with Dactarin Gel for baby and mum, applied after every
feed. There was not much improvement so we moved to Dactacort for mum and
Gel for baby. Still not much improvement so we moved to oral fluconozole
with dramatic improvement for mother the stabbing pains abated but she
still gets "sand paper" feeling at the end of feeds but it is nothing like
it was. Baby's tongue is still coated white. It has been about three
weeks of treatment with the fluconozole introduced on the second week for
mum only. I have had the mother look out for Raynaud’s symptoms and she
states that her nipple turn red but never bluish and she only ever saw them
white one time and they didn't hurt then. But even if it is Raynaud’s that
wouldn't explain the baby’s tongue. Any help would be appreciated.
