In the posts about this case so far, no one has asked how much below
birthweight this baby is, so I will do so now.  I'd also like to know what
birthweight and length were, head circumference, and what was the lowest
weight and when did it occur?  How long did it take before baby started
gaining?

A baby who is still a lot under birthweight after more than 3 weeks, needs
help fast.  If the baby was born skinny, this could be even more serious.  I
will paraphrase Barbara Wilson-Clay from several posts by her on this topic
in the past - a baby who is not getting enough nourishment is not going to
be good at feeding.  If the situation has persisted for this long, the baby
could be losing patience.  

There is still no reason for a bottle, but depending on the rate of gain
under the current plan, there may be good reason to increase the amount of
supplement the baby is getting.  It is so much easier to work with a baby
who is getting what it wants, namely enough food to grow on.
