No consent form. At Georgetown Un. Hosp. we have a Nipple Shield care plan 
that is given to Mom if discharged with a NS.  The need for FU care is a 
"line item", she is offered a FU appointment in our Lactation Center or if 
she chooses to see her own LC we indicate on the care plan when she should 
receive FU care.
