Newborn screening here in New England is done after 24 but preferably before 72 hours, and here in Vermont we're testing for ten different things with one heelstick. You can read a great deal about this at http://www.umassmed.edu/nbs/   
 
In a diverse genetic population (and a litigious one at that!) there may be a need to do a greater array of tests. I know Cheston Berlin has shared some of his experiences working with a population where there's a high incidence of PKU -- I've never seen one. But we do get families who specifically request others, and those can be added on at no extra cost.
