I have seen women start having pain around 3 weeks with vasospasm...sharp 
pains into breast after feeding...may not feel it in the nipple itself. 
Often mom has lots of milk, and baby pinches during let-down to slow the 
fire-hose.  Or latch was poor---but spasm persists after it is "fixed".  If 
you do not stop the spasm, it can keep iteslf going...spasm causing 
continued tissue damage by not allowing enough oxygen to the tissues.

So...I would have her try a heating pad on "low" applied immediately after 
feedings.  Do not let nipples air dry, but cover up as soon as possible, and 
apply heat to try and keep the spasm from happening.  And conisider the 
possibility of over-supply or just very strong let-down.
