I am beginning to think about creating a good form which I can use to chart
clients that will be acceptable to physicians, maybe something close to what
an RN or a hospital would use. I am thinking rather than completely
re-creating the wheel I would ask what other people are using, and if they
have something they could email me as a WORD or EXCEL attachment? The basic
LLL call documentation form is not detailed enough for me; I was wondering
if anyone has a flow sheet or check-off sheet so they tend not to miss
common issues? I may not be using all the right words here, especially
since I am not entirely certain what I want myself . sorry!
