i believe it is possible for the mom to have few nipple opening on the 
Nipple. she may have abnormal breast development. if she is 3 days pp and 
getting engorged and you are not seeing good results w/ pumping, i would try 
the reverse pressure softening and hand expression. you mighty try cold 
cabbage leave therapy or just cold therapy. i bet the mom is getting very 
anxious so try some relaxation, whatever works for her maybe warm towels 
around her and a backrub. or some hot tea.
i would also evaluate the baby closely and the milk transfer as this baby 
may need to have some additional milk until this resolves, if it does.
