In reply to Jane's questions about nutrition, albumin, pre-albumin and how they influence fluid status. Briefly:
Albumin does regulate fluid in the intravascular space, and it does drop from malnutrition and excessive IV fluids resulting in fluids shifting to the extra-vascular space (3rd spacing). Unfortunately, nutrition cannot fix the problem of excessive IV fluids, the kidneys just have to get to work.
Albumin has a half life of about 20 days, so eating protein would not change your albumin levels fast enough to make a difference. The albumin level will go back to normal once the extra fluids are cleared by the kidneys - not vice versa unless IV albumin is given.
Pre-albumin is the nutritional lab of choice because it has a much shorter half life and is not affected by hydration like albumin is, so you can detect nutritional (energy and protein) deficiencies and repletion within a few days. Albumin on the other hand takes a month or so to recover from a nutritional deficiency.
So, unfortunately, nutrition does not help us here.
PS. On a previous post I spoke about the Registered Dietitian vs. the nutritionist. I forgot this is an international forum, and RD is a credential available mostly in US. My apologies to the other nutrition professionals, I truly meant to say that you should get your nutrition info from a professional in that field, whatever that means where you are practicing.
