This sounds like a nipple bleb.  Every time I see one I tell the mom I wish 
someone would biopsy one so we would REALLY know what they are.  If this mom 
does have a biopsy, please ask her permission to share the pathology report. 
I would LOVE to read it.  My personal favorite theory is that it is the 
side-wall of a milk duct sticking out with milk trapped behind it.  I doubt 
it is a malignancy, but without seeing it, would not tell her to skip the 
recommended biopsy.  A biopsy should not be disastrous, but would be 
painful.  If the bleb has been there a long time, you don't always get any 
milk out of it when lanced.
