I have worked with one such baby...

A tube at the breast did not work, because the problem usually is they 
cannot generate suction well.  (Unless mom can manage pushing a 
syringe/feeding tube set-up herself.)  As I recall this baby got expressed 
milk in a Haberman bottle then nursed afterwards, because removal at the 
breast was very poor.  As with other structural issues, I assume the 
shape/size/consistancy of mom's breast tissue can make a difference in how 
well milk is removed.
