That Mom will need lots of encouragement and watch the baby's weight. Even
if you hear some swallowing the baby may not always be able to take in the
whole nipple. In that case the mother's supply must also be watched. She
will probably need to pump and top baby up with whatever device she is
willing to use for a few weeks until baby catches up to the nipple size.

I've always tried to add some humour and tell them that it  is actually a
syndrome called baby booble disproportion. That generally relaxes them some.
Especially when I tell them that I see this situation quite frequently. A
tincture of time will be needed. How much time depends on the disproportion
and the type of nipple if they are quite thick and fibrous it may take a bit
longer but if they are soft but large nipples they usually get on pretty
quickly.

I always have the Moms try for a bit each feed but not to overdo it if there
is a real disproportion. You don't need sore nipples as well as the time it
takes to pump etc. I know that some Lactation Consultants go right for the
Lactation device and formula with no pumping. But that way the mother
usually ends up with a low supply just when she could totally breastfeed.

Having the baby lay almost over the breast as in the video about 'self
attachment"often seems to be the least uncomfortable.
