Dear Colleagues:
I am currently dealing with a mother (6 wks. PP) who was hospitalized with
a urinary tract infection, temp. of 103 upon admission, and bilateral
mastitis. The baby is on Nystatin for thrush. Mom had been on Keflex
for UTI symptoms since 7/26. The UTI was probably related to a
ureterostomy stent that she has had in for about one year. She was
hospitalized for removal of the stent, but her urologist did not want to
remove it until the UTI was treated and temperature resolved. The urine
culture on day 4 of hospitalization showed vancomycin-resistant
enterococcus faecium and pseudomonas aeruginosa. Her infection control Dr.
put her on Linezolid and Maxipime. She went home after removal of the
stent with a PICC line in and is to continue on these antibiotics for 2 - 4
weeks. She was advised not to breastfeed until the Linezolid has been
discontinued for 24 hours.
By the way, she had her baby with her in the hospital and nursed until
started on the Linezolid. This was the only way she would consent to being
hospitalized.
It made her caregivers very uncomfortable to have her baby exposed to the
VRE.
Do any of you have experience dealing with a situation like this? If so, I
am most anxious to dialogue with you to learn how you managed the situation
and what was the outcome.
