My name is Cathy McLendon, I am an IBCLC from Chico CA. I have been asked 
to draft an informed consent for formula supplementation to be used at our 
hospital. This request came from the Pediatricians who were concerned about 
unnecessary supplementation occuring especially at night. I know that over 
the years I have seen many samples of these but of course now that I need 
it I am unable to put my hands on one. If possible I would greatly 
appreciate input from facilities who currently utilize a consent.
