Having worked with many a mother with large nipples, I was glad to see that
Medela now makes a 36 mm plastic flange, so there is something between the 30
mm and the 40 mm glass flange.
On the breastfeeding aspect of the topic, I have used nipple shields to
help an infant fit enough breast tissue in the mouth to facilitate effective
feedings, while baby grows.
