Judy, I noted a couple things in your post.  You described the baby  nursing 
fine in the first 5 - 10 minutes taking in 2-3 oz and then you go on to  say 
the baby then comes off the breast coughing and choking and the rest of the  
feeding takes 45 min.
How about along with some of the other approaches you are trying, to also  
stop the feeding when the baby starts to have problems and wait awhile.   Small 
frequent feeds may be better and easier on this baby and mother.
If you can get a hold of a copy of Wolf and Glass, their book may have some  
helpful insight into your case.
I agree that mom's supply needs to be brought down and to try positions  that 
allow the baby to take the milk in at a slower pace, such as  side-lie.
