I have a client with permission to post that has a persistant hard nipple
that originally looked to have a plugged milk duct.  She is a 30yo, 2nd
time BF a 4 month old.  On examination, she appeared to have several
Montgomery glands close to the nipple that she thoutgh were plugged ducts
and was concerned that she could only see about 5-6 nipple pores(we
discussed this) There was one very obvious plug that was slightly
irritated, from sqeezing, according to mom.  She used a sterile needle to
put a small punture and release some milk she also used warm compresses
also started Lethithin.  The nipple is painful to touch and more painful
when nursing, regardless of positioning.  There is no blanching to the
nipple and no obvious hardened areas in the breast. I have never heard of a
nipple staying hard with a plugged duct and it doesn't feel to have a
cyst.  Any help would be appreciated!
