For the baby with weight gain issues, I would suggest investigating reflux.
I am seeing so much of it, some with babies gaining *too much* because the
reflux after the feeding causes fussiness and in response mom puts them back
to the breast.  More milk.  More fussing.  More feeding.  Others don't gain
well enough because the discomfort causes them to be fussy feeders and come
off, go on, come off and so on.  I usually see tight latches, wherein baby
tries to cope with the reflux and still be able to breastfeed. In this case,
the fussiness often accompanies let down.  Just a thought.
