This business about bottlefeeding being less tiring or otherwise better for
premies and sick newborns is nonsense! Here is some evidence to support
breastfeeding. If you look at Brian Palmer's website there are some interesting
points about breastfeeding. They are not specifically for premies but I think
it's relevant. The first link is a slide presentation, comments are in the
second link.
Some points I gleaned from this. "During breastfeeding the larynx elevates
which allows the epiglottic to interlock with the soft palate. This allows the
newborn to breath and swallow at the same time." Can't do that with a
bottle. I have seen many premature babies that could not coordinate the suck,
swallow, breathe with the bottle and would have A's and B's. He also states that
stronger suction is required during bottlefeeding, and that the vacuum created
by sucking on a bottle causes inward collapse of the throat, oral cavity, and
airway. Yikes!
There is also an interesting point which supports the argument that frequent
breastfeeding during the night (such as a baby sharing bed with mother would
do) protects against SIDS. "As the tongue drops back in the mouth, the
epiglottis has to drop down. As distance between the epiglottis and soft palate
increases, the tongue base acquires a position where it can drop back and block
off the airway."
