I take a similar approach to Laurie Wheeler's. If I have an underfed
newborn, I use the 2.5 oz/lb body weight for needed intake calculation,
up to about 6 weeks of age. After that, I use the average intake based
on gender (24-28 oz for girls, and 26-40 oz for boys) as the target
intake. The boy range is really wide, so I look at the parents. If
they are tiny, I suggest near the beginning of the range, and if they
are tall and the baby is long, I suggest 32 oz or more, and we refine
based on weight gain. I find that intake goals are especially important
for parents of premies/near premies, SGA babies, and babies with
neurological issues (Down syn, etc) that would make them unusually
sleepy or passive. Term babies tend to advocate for themselves more
effectively when they are underfed, up till the point they go on "energy
conservation mode".
The other numerical calculation that I use all the time is that it takes
2 oz per day to gain 1 oz per week. When we have a baby who is not
gaining well, we can calculate needed supplement this way.
