You need to have your NICU manager talk with your billing people.  If, as in 
California, there is a per diem rate for each hopsital day, it will include 
the cost of the donor milk (and you will need to "eat" the cost of the milk, 
although saving even one day of hospitalization will pay for all of it - see my 
article J Perinatol 2001).  If you are still on the old fee-for service system 
and your hospital bills separately for each service, you just add it to the 
patient's hospital bill.
