I am working with a first time mother who is struggling with low milk
supply. I started working with her when her baby was 8 weeks old. At that point
she already had low milk supply, one severely damaged nipple and one slightly
damaged nipple, baby had gained one pound over birth weight, supplemented for
five days with formula and returned to breastfeeding.
She has, in the two weeks we have been working together, rented a Lactina
(pumps 8-10 times or more/ 24 hours), tried SNS (baby figured out milk came from
tube and let go of nipple after 24 hours), nipple shield (baby HATED it),
she is now finger feeding and breastfeeding on least damage side alternately
throughout each feed. Supplementing pumped milk with ABM. Baby's weight gain
is dramatically improved and she went from sleeping, content baby to active
(i.e. normal) baby but she still falls asleep at the breast if she isn't
REALLY hungry. Mom has been taken More Milk Plus for almost one week. Took
Fenugreek for one week with no changes in milk production or breast fullness.
MMP has increased her breast fullness but only increased milk production by
about 1/4 to 1/2 oz. per pumping session. She is pumping between 1.5 and 2 oz
each session with breast compression during pumpings.
Baby's latch has dramatically improved from two weeks ago when I first saw
them nurse. She was compressing the corners of her mouth making mom's nipple
into lipstick. She now only does this when the milk flow slows and not as
dramatically as before.
I WISH we had a CST available here in El Paso. I have just found out that
someone comes in from Tucson once each month. I have been pulling my hair out
on this one because I really think that this baby is a PERFECT candidate for
CST. I have mentioned it on several occasions to mom and she said the last
time that it sounds "scary"...I have to agree if it were my baby and I had
never heard of CST that it would indeed be scary. But it was a moot point
because there was no one to refer her to at that point. Hopefully I can track
this person from Tucson down and get a referral into her.
Mom's problem is obviously milk supply; what are we missing here? I have
talked to her about getting hormone levels checked. No problems with thyroid
or fertility. One breast is larger than the other and produces more milk.
Other than that, no obviously physical indications of supply problems.
Within the past 24 hours baby has been less willing to latch and work; mom
is concerned that they are moving away from nursing now instead of back to
breast. This concerns me also because this baby picks up on "new tricks" VERY
quickly and I don't want her to not go back to breast fully because of
something I am not seeing/working on right now.
What else can we try?
