I have seen self-limiting refluxers - babies with very painful reflux
who limited their feedings at breast to an ounce or so at a time,
refusing to take more milk for awhile, despite ample maternal milk
supply (one mom pumped 3 more oz from the same breast after this baby
came off after exactly 1 oz). When this same little gal was forcefed a
bottle of formula in the pedi's office (pedi said "I'll *make* her eat")
she shut down and refused to eat orally again for months. Had to be
hospitalized and tube fed.
Wolf and Glass mention a micrognathic infant who was FTT on bottles of
abm, due to placement of the nipple under the tongue with very poor milk
transfer. I've also seen kids who were poor bottle feeders and could not
meet their needs at either breast or bottle.
