David you have lots of good questions, ....it is amazing how many women who  
have had a previous breast surgery (reduction and augmention) do not tell 
their  OB or hospital staff, ....how often, more so lately, I find myself  feeling 
the need to inquire, and casually asking during the breast exam  "any 
previous breast surgery?", .....these operative procedures used to be easy  to spot 
at first glance, but are getting harder to spot easily, and I might not  ask if 
not for the hint perceived from palpation, ...and how often the response  is 
"oh yes, but that was years ago" as if it should not be an issue by this  
point.
 
yes I give additional education to these women, ....an excellent resource  is 
a book and website by Diana West, ....as a fellow member of Lactation  
Consultants Association of Greater Washington, Diana spoke at one of our  quarterly 
meetings last year: her presentation was fabulous!  .......see BFAR.org
