Good Morning all,

I work in a hospital and am working with a mom and baby born 12/17/04.
(Permission to post) Birth weight was 6-6, discharge 6-4. After birth I
was not able to see this baby latched because she would not stay awake.
Discharged the next day. Readmitted  day 7 for jaundice and weight loss
of 11 1/2 oz. My partner saw her then, and baby was still unable to stay
awake. Mom was pumping with a hospital grade pump. At that visit, mom
stated a tender right nipple. The usual comfort techniques were
reviewed. After discharge, many phone calls with latch/pain issues. 

Finally able to have mom come back in. Digital exam revealed a deep
palate, tongue up and back. She would bring her tongue out to cup my
finger, but would then quickly retract it again. After much positioning,
able to get baby on without pain on left breast. (Baby did not like
being held in close, and fought the breast). Moved her over to the right
and noticed that her left nipple had a white undertone. After feeding on
right, nipple was white (not a bleb) so we discussed the pain she has
after the feeding which throbbed and lasted a few hours post feed. She
and I both think its Raynauds. Mom started Vit B6 that day. Phone call
today with mom unable to latch again without pain. Did the finger
teasing over the weekend to help draw that tongue down and out. Mom very
frustrated, she really wants to breastfeed.  So mom is now pumping to
rest her nipples. 

If she can get the Raynaud's under control, any suggestions to get her
baby's tongue down and out? There is a chiropractor who does CST, would
this be a route to go? Or an OT?  

I appreciate any advise y'all might have, I really feel for her. I will
speak to her again Wed., and if the Vit B6 isn't helping, will call her
OB for Nifidipine.
