Just wanted to thank all who have responded.  I am now heavily armed with
written info!!!!!  I have just drafted a letter to my health plan with the
help of my LLL Professional Liaison and a fellow LLL Leader who happens to
be an LC for my health plan (she's also going to get in touch w/them for
me!).  So I'll let you know what the response is to the letter.  Thanks
