I will combine 2 replies so as not to go over my lact-limit for the day ;)

The lady with the breast pain could try: 1. culture the milk for mastitis 
(pain and clumps), 2. treat for mastitis (just based on the signs/symptoms) 
with antibiotic and anti-inflammatory and frequent emptying, 3. treat for 
vasospasm (blanching) with nifedipine and/or anti-depressant type meds which 
are sometimes used for 'nerve pain' or neuropathy, and/or use pain meds 4. 
CST or chiropractic for infant suck/clamping. 5. express only from that 
side, or unilateral weaning.

The lady with the crustiness on the nipple could easily culture that and 
yeast buds should be seen microscopically if there. If not, there have been 
posts in our archives about moms gently debriding or using oil to soften 
what appear to be layers or plaques of skin.
