If a NP is going to do lactation consultations, in order to attempt to get
decent reimbursement you need to be sure to bill based on TIME spent. In
the E and M coding system there is a "clause" stating that if >50% of your
time with the patient is spent "counseling", then you may code based on
time. (see CPT manual) The other tip is to bill for both the mother and the
baby as individual patients if they have separate problems (ie mastitis and
wt loss) BUT if you do this you have to be careful to document all needed
info. (HPI, PMH, FH, etc)
