Dear Pamela,
First I would think about the weight gain. 1 lbs 2.5 oz since birth (at 3
weeks). When did baby regain birth weight? I would look at Diane
Wiessinger's handout, "Gaining, Gulping, & Grimacing" because it really
does seem to describe this baby. I have dealt with this a lot in my
relatively short time in private practice and most moms do at first believe
that the baby is allergic to something in their milk or that the baby isn't
getting enough. See http://www.wiessinger.baka.com/bfing/trouble/gulping.html
The best way to rule this out is to actually observe a feeding and
determine milk transfer for that feeding and have mom pump afterwards to
see how much milk she is producing. So say a baby transfers 4 oz at a
feeding and mom can pump an additional 4 oz after. And I've seen moms able
to pump 8 oz after transferring 4 oz! Does mom make a point of feeding
from both breasts at each feeding?
Anyway, this is something that is relatively easy to fix (certainly before
diet food elimination trials) and fairly easy to rule out or in. If this
isn't the problem then I'd go on to look at diet.
Hope this helps,
Cindi Swisher, RN, IBCLC
