I have a few questions:

I see you put "breastfeeding well" in quotes, so did mom give you some info
that baby was, in fact, not feeding well from the beginning?
What was wt of baby at hospital discharge? Were there other wts within the
13 days? What were they?
Any risk factors in infant? Birth trauma, injury? Jaundice?
What were the "diaper diaries" like in the early days and later on?

Did mom's milk come in fully around day 3-5?
Is mom on any meds? I would be specifically interested in hormonal meds like
birth control pills or shots; cold meds?

What I would be trying to determine is if infant was compromised in some way
from the beginning or did infant deteriorate at some later point? If "by day
12 baby was not nursing as well" but he was nursing well before that, that
sounds like a sign of a serious illness that needs close investigation. In
any case, at this point, feed the baby, maintain/increase the milk supply,
closely observe infant for illness or developmental abnormality, work on the
breastfeeding skills, refer if necessary to OT or other provider. Good luck.

<<have a mother of a two week 8 lbs 7oz at birth. Infant was sent home
"breastfeeding well".
By day 12 baby was not nursing as well and she began pumping after nursing
and giving infant EBM by bottle (Avent slow flow nipple). At 13 day MD appt
infant had lost a pound. >>
