Hi, Cathy: Talk to your L&D people and find out what company they order
their delivery logbook from. We use:

They will custom-make a lactation phone log for you. Ours is a hard-back
ledger-size book with LACTATION PHONE LOG and the hospital's name printed
in gold on the front. There are 8 columns on each page: DATE, TIME,
CALLER, PHONE NUMBER, BABY'S AGE, CONCERNS, INTERVENTION, FOLLOW-UP. They
design the page with whatever information you provide. We started our
current one Jan. 1, 2003 and we're not even halfway thru it, so there is
plenty of space. And it satisfies Joint Commission documentation
requirements.
