Despite the cardiologist's assurance, I would ask him to check baby Kyra out
while feeding - her response to fast flow makes me concerned that she is
struggling with breathing or her heart while coordinating
suck/swallow/breathe.  A slow flow bottle nipple might help.

It's really hard when an experienced mom like this is struggling - you know
the answers aren't the easy ones :-)
