Christine, I reread your post three times looking for the problem and
couldn't find it. Baby is "gaining well, never appears to be in pain or
uncomfortable," just spits up. In this situation I don't see the need
to have her start taking any supplements or try to talk the baby into
nursing more frequently. One green stool in a happy baby means
nothing. Sure, her diet could be better and less stress would be good,
but those factors don't seem to be causing any breastfeeding problems.
About the only thing I would tell her is to not change sides immediately
if baby comes off the first breast for a break, to go ahead and put him
back on the same side after a burp. That may keep him from taking in
too much milk for his tummy. If the spitting gets really bad, she could
do two consecutive feedings on the same side, but I would let him decide
when he wants to feed. And she should keep lots of burp rags around. :-)
