I would be interested to know how long the piece was, and where it was
placed. I always use one very long piece (~3" or more) that goes part way
onto the nipple, and then an anchor piece that is way away from where baby's
mouth will be. I've never had a problem, and I can't imagine one unless the
tape were somehow able to dissolve and rip off, which also seems very
improbable.
