Thanks to everyone who has responded re this baby.  It is wonderful to have
this resource in which to bounce ideas and worries.

Update:
The mother saw NP at the clinic today.  She gave me the # of the clinic and
I called and got the fax # and faxed my report so that the NP would have it
when she saw the baby.  According to the grandmother (mother was at a class
this evening), the NP gave her a referral to an SLP to assess the
feeding.  So hopefully they can get an appointment for that soon.  The
grandmother said that the mother was very pleased with the NP and felt that
the NP listened to her -- that she agreed that the tongue could be causing
the problems.  She said that the NP didn't seem to think reflux was the
problem, but did start the baby on some reflux med.

The mother told me this morning that the father was able to get the baby to
take an ounce from the bottle with a medium amount of effort this
morning.  (just to give mom a break -- remember, she's been in pain for
over 4 weeks).  Mom stated that cutting off the flow with a scissor hold (I
forgot to mention that I taught her that as well) worked very well and had
the baby staying longer at the breast, without arching.  Still painful
though.  They are still doing the tongue exercises and pacifier tug of
war.  Maybe seeing a slight improvement, but not much to notice at this point.

Anyway, thanks again.  I'll give an update when I know more.
