I am interested in the perspective of others.

I have a close family member who recently gave birth.
primip. 25 y/o no complicating health issues, normal pregnancy. full term.
no labour. What we locally call 'good equipment' large soft breasts mid
sized protruding nipples.
on due date she developed bleeding . infant was not in distress. C section
was performed due to placental separation.
pathology report shows the placenta had separated in a donut shape with a
large clot in the middle. Her hemoglobin and blood pressure did not change
until the very last so it is suspected this did not take place until the
day or less prior to the exterior tear which led to symptomatic bleeding.
Placenta was 90 - 95% detached. Narcotics were discontinued in 24 hours,
mom was on suppository anti inflammatory (name forgotten sorry) and
occasional extra strength acetaminaphen

The early separation was calculated to be less than 50% of the placenta the
later symptomatic tear immediately before delivery 25% of the
placenta  This was a very alert, relaxed and cooperative infant who was
offered the breast frequently. She nursed immediately after delivery,
nursed well again at 8 hours and with little difficulty and no pain several
times during first 48 hours.Mom was relaxed, had midwife, doula and LC
support throughout. Family remained in hospital to provide 24/7
support.  Change from colostrum was noted at approx. 30 hours leaking by 48
with obvious copious milk supply. No engorgement but emotionally volatile
at the end of day 4.

Question:  Do you think the partial separation of the placenta could have
induced the milk supply so that it came in on day 3 after the separation
not day 2 after birth?? However that does not explain the emotional
volatility which arrived on schedule. I would have suspected that enough of
the placenta was intact to prevent induction of lactation given how little
a placental fragment can suppress lactation in other women. Also some of
the literature suggests a C section would delay milk by 24 hours...

What has been the experience of others with mothers with placental separation?
